Insider Threat Incidents: What the First 72 Hours Actually Look Like

When the threat comes from inside, the playbook most teams have prepared is almost useless.

What makes an insider threat incident different from an external breach?

An insider threat incident is fundamentally different because the attacker has legitimate access. There is no perimeter to defend. The credentials are real, the behaviour patterns look normal, and the usual indicators your SIEM was tuned to catch simply do not fire. By the time anomaly detection surfaces something, the damage is often measured in weeks of undetected activity, not hours.

LimitedView's analysis across 847 organisations shows that insider threat incidents take an average of 47 days longer to contain than external breaches. The reason is not technology. It is the human layer: delayed reporting, internal politics, and a workforce with no training context for what to do when a colleague is suspected.

What should the security team actually do in the first 24 hours?

In the first 24 hours, contain access without alerting the subject, preserve evidence, and loop in legal and HR immediately. Most security teams get two of those three right. The third is where investigations unravel.

The instinct is to lock accounts and confront. That instinct costs investigations. A malicious insider who knows they have been detected will either delete data or claim victimisation. Neither outcome is clean. The first 24 hours should be about silent observation: elevated logging on the account, network traffic capture, and a documented audit trail that will hold up to legal scrutiny.

Your IR playbook almost certainly does not have a separate workflow for suspected insiders. It should. The steps for containment are inverted compared to an external attack. You preserve access to watch, rather than cutting it to stop bleeding.

How does the wider workforce respond, and why does that matter?

Workforce response in insider threat incidents is rarely discussed in incident response planning, but it shapes outcomes significantly. When colleagues learn that a peer is under investigation, behaviour changes. People get nervous about what they have shared, what they have clicked, what they have forwarded. Some will attempt to "help" by destroying what they think is evidence. Others will tip off the subject directly.

In LimitedView's data, organisations with incident-triggered training had a 64% reduction in secondary incidents triggered by workforce disruption during an active investigation. The mechanism is straightforward: employees who have received context-relevant training know what to do and, more importantly, what not to do. That is not an accident. It is the product of training delivered at a moment when relevance was obvious to everyone receiving it.

What training gap does an insider threat incident expose?

The gap it exposes is that most security awareness programmes treat employees as potential victims of external threats. That is a reasonable frame for phishing. It is the wrong frame entirely for insider risk.

Employees need to understand their own data handling, their own access hygiene, and their own reporting obligations. They need to know that reporting anomalous behaviour from a colleague is a professional responsibility, not a social betrayal. That message requires more than a slide in annual compliance training. It requires context, and context is most effectively absorbed immediately after a relevant event.

The organisations in LimitedView's dataset that had experienced at least one prior insider incident showed 73% retention of reporting protocols six months after incident-triggered training, compared to 12% for those who had covered the same content in scheduled annual programmes. The difference is timing, not content.

How do you communicate with the wider organisation during an investigation?

During an active insider threat investigation, say as little as possible to as few people as possible, for as long as possible. That is not comfortable advice. Boards want updates, managers want explanations, and employees notice when something is wrong.

The communication approach needs to be tiered. Incident response leadership knows everything. Senior stakeholders get a need-to-know summary, framed around operational continuity rather than investigation status. The wider workforce should hear nothing specific until the investigation is concluded and legal has cleared the communications.

What they should receive, once it is safe to do so, is honest context. Not blame, not procedural sanitisation, but a factual account of what happened and what the organisation is doing about it. That communication, delivered within days of resolution rather than weeks, is the single most effective thing a CISO can do to rebuild trust and reinforce the reporting behaviours that will matter in the next incident.

What does a post-incident review need to cover?

A post-incident review for an insider threat case needs to address three things: how access was granted in the first place, how long it persisted without scrutiny, and what would have detected it sooner.

Least privilege is the technical answer to the first two. But LimitedView's analysis consistently shows that technical controls alone do not close the gap. Organisations with strong access hygiene policies but weak security culture still took an average of 38 days to contain insider incidents. The detection shortfall is human. People see anomalous behaviour and do not report it, because they are not sure it matters, or because they do not want to be wrong about a colleague.

That is a culture problem, and culture is built between incidents, not during them. The organisations that recover fastest are those that had already built the muscle of honest, low-friction reporting before the incident arrived.
